Patients ready to be discharged from the hospital aren't always ready to go home. Lutheran Health Network provides different levels of short-term, post-acute care for patients who need extra recovery time, skilled nursing care and rehabilitation services. The level of care required varies based on each patient's ongoing needs.
Skilled Nursing Care
Skilled care services help patients who are recovering from illness, injury or surgery transition from hospital to home. This is achieved through:
- Individualized care plans
- 24/7 registered nursing care
- IV therapy
- Respiratory therapy
- Physical, occupational and speech therapy
- Daily activities
- Lab and radiology services as needed
- Discharge planning and social services
Inpatient Rehabilitation Facility (IRF)
The Rehabilitation Hospital of Fort Wayne is the only hospital in the region dedicated solely to physical medicine and rehabilitation. Patients who have experienced a disabling injury or illness receive focused care to help them reach their maximum potential and regain independence.
Benefits of inpatient rehabilitation facilities include:
- Ability to manage complex medical conditions

- Minimum of 15 hours of therapy per week
