Patients & Visitors
Current Visitor Guidelines (effective 03/03/21)
For your safety, as well as the safety of patients and caregivers:
- Face masks are required at all times.
- All visitors must be 18 years of age or older and will be screened upon entry.
- Anyone with COVID-19 symptoms will not be permitted to enter unless seeking medical care.
- All visitors are asked to practice social distancing, hand hygiene and infection prevention:
- Sanitize hands when entering the facility and when entering or exiting patient rooms.
- Cover coughs or sneezes with elbow or a tissue and dispose of used tissues in the trash.
- Sanitize hands after coughing or sneezing.

Hospitals
- One visitor: Only one visitor per patient at a time
- Visiting hours: 8 a.m.-8 p.m. daily (see exceptions below).
- Inpatient visitors: Go directly to the patient room. Cafeterias and gift shops are closed for visitors.
- Outpatient procedures/surgery: One person may accompany each patient (see exception below).
- Radiology and Outpatient Diagnostics: One person may accompany each patient.
Exceptions:
- Emergency Department: One person per patient (no visiting hour restrictions). Both parents/guardians are permitted for pediatric patients.
- The Birthplace: Two designated visitors (including spouse or partner); only one adult visitor per patient at a time (no visiting hour restrictions).
- Pediatrics and NICU: Two parents/guardians (no visiting hour restrictions).
- Compassionate care/end-of-life situations: Please speak with a member of the care team as more than one visitor at a time may be arranged.
- The Orthopedic Hospital Outpatient Surgery Center (7601 W. Jefferson Blvd.): Visitors are unable to accompany patients at this time.
No visitors permitted:
- No visitors for COVID-19 patients or for patients awaiting COVID-19 test results.
- No visitors are permitted in continuing care or behavioral health units.
Lutheran Health Physicians Practice Locations
- One visitor may accompany each patient.
RediMed and MedStat Urgent Care Locations
- One visitor may accompany each patient.
The Rehabilitation Hospital of Fort Wayne
- Only one visitor per patient at a time.
- Visiting hours are 9 a.m.-4 p.m. daily.
- Please go directly to the patient’s room or designated visiting area.
- No visitors permitted during therapy sessions. (Check the patient schedule daily for therapy times.)
This is an extraordinary time and we appreciate your understanding and compliance.

Advance Directives
Each individual has the right to direct his or her own care. Current information about Advanced Directives and Indiana state forms can be found on the Indiana State Department of Health website.
Forms included, but not limited to, include:
- Health care representative appointment
- Life prolonging procedures declaration
- Living will declaration (English and Spanish versions)
- Out-of-hospital do not resuscitate declaration and order
Parental Consent
If you're traveling and have to leave your children in the care of someone else, be sure they're protected. Always leave written authorization so your children can receive medical treatment, if needed.
Simply download the form below, fill it out, and leave it with your children's caregiver. An authorized form is valid for one year following the signature date unless the form has been withdrawn with written consent.
