Patients & Visitors
Current Visitor Guidelines (effective 11/23/20)
For your safety, as well as the safety of patients and caregivers:
- Face masks are required at all times.
- All visitors must be 18 years of age or older and will be screened upon entry.
- Anyone with COVID-19 symptoms will not be permitted to enter unless seeking medical care.

Hospitals
- One visitor per day: Only one visitor is permitted per patient per day.
- Visiting hours: Noon-8 p.m. daily. (See exceptions below.)
- Inpatient visitors: Go directly to the patient room.
- Outpatient procedures/surgery: No visitors are permitted unless the patient is a minor or needs special assistance. Patients must be dropped off – the person driving the patient will be given a time to return to the post-op waiting area.
- Radiology and Outpatient Diagnostics: Patients must be dropped off. One person is permitted if the patient is a minor or needs special assistance.
Exceptions:
- Kosciusko Community Hospital: Visiting hours are 10 a.m.-noon and 6-8 p.m. daily.
- Emergency Department: One person per patient (no visiting hour restrictions). Both parents/guardians are permitted for pediatric patients.
- The Birthplace: Two designated visitors (including spouse or partner); only one adult visitor per patient at a time (no visiting hour restrictions).
- Pediatrics and NICU: Two parents/guardians (no visiting hour restrictions).
- Compassionate care/end-of-life situations: Please speak with a member of the care team.
No visitors permitted:
- No visitors for COVID-19 patients or for patients awaiting COVID-19 test results.
- No visitors are permitted at The Rehabilitation Hospital, The Orthopedic Hospital, or in Continuing Care and Behavioral Health Units.
Lutheran Health Physicians Practice Locations
- No visitors permitted unless the patient is a minor or needs special assistance.
- One visitor is permitted for OB patients.
Redimed and Medstat Urgent Care Locations
- No visitors permitted unless the patient is a minor or needs special assistance.
- One visitor is permitted for OB patients.
This is an extraordinary time and we appreciate your understanding and compliance.

To make your experience easier, we want to provide you with helpful information you may need to plan your visit, such as what to expect, parking instructions, transportation and more.
Advance Directives
Each individual has the right to direct his or her own care. Current information about Advanced Directives and Indiana state forms can be found on the Indiana State Department of Health website.
Forms included, but not limited to, include:
- Health care representative appointment
- Life prolonging procedures declaration
- Living will declaration (English and Spanish versions)
- Out-of-hospital do not resuscitate declaration and order
Parental Consent
If you're traveling and have to leave your children in the care of someone else, be sure they're protected. Always leave written authorization so your children can receive medical treatment, if needed.
Simply download the form below, fill it out, and leave it with your children's caregiver. An authorized form is valid for one year following the signature date unless the form has been withdrawn with written consent.
