- Review details of a hospital stay, including the reason for admission, as well as procedures, results and discharge summary

Frequently Asked Questions
Important details:
- Patients can enroll in MyHealthHome during an office visit or hospital stay or self-enroll once home. Dependent children and adults must be enrolled in person.
- Because each patient's medical record can be securely accessed at any LHN or LHP location, portal users only need to enroll in the MyHealthHome portal once.
- Patients can still access the previous patient portal (powered by FollowMyHealth) for details about visits and services prior to the conversion dates listed below:

Access MyHealthHome with Apple Health
Our hospital and providers now support Health Records on iPhone, which brings together hospitals, clinics and the existing Apple Health app to make it easy for patients to see their available medical data from multiple providers. Learn more about how you can view your encrypted, secure health records on your iPhone. Patients must be enrolled in MyHealthHome before they are able to connect through Apple Health.
