Lutheran Health Quality Alliance (LHQA) is a physician-driven collaboration among members of community practices across northeastern Indiana and Lutheran Health Network. The program employs evidence-based clinical quality guidelines and will actively monitor the performance and outcomes of its participating providers to assure that high-quality, cost-effective care is being delivered.
The goal of the Lutheran Health Quality Alliance is to deliver high-value healthcare services to community members and to become a leader in value-based delivery arrangements in the northeastern Indiana marketplace.
Implementation of the organization’s population health data management tool - a web-based data repository and reporting system - plays a vital role in helping the network achieve its goals. This tool will be used to support the analysis and reporting of metrics associated with the clinical integration program.
