Summit Home Health Changes Its Name to Lutheran Health Network Home Health
FORT WAYNE, Ind. (Friday, Jan. 25, 2013) – An affiliate home care service of Lutheran Health Network, Summit Home Health, will now operate as Lutheran Health Network Home Health. The name change took effect Jan. 1, 2013, in an effort to more clearly align the agency with the Lutheran network of healthcare facilities and services.
The agency’s main office is located at 2510 E. Dupont Road, Suite 237, in Fort Wayne. Services extend to nine counties — Adams, Allen, Blackford, DeKalb, Huntington, Jay, Noble, Wells and Whitley. The agency can be reached by calling 260.490.6535.
Home healthcare allows patients who are elderly, disabled, recovering from a surgery or illness, or chronically or terminally ill to receive care in their own homes. Services include skilled nursing, as well as physical, occupational and speech therapy. Medicare, Medicaid and most commercial insurance plans cover home health and palliative care services.
Lutheran Health Network Home Health supports facilities’ efforts to achieve the best possible patient outcomes, particularly after discharge. Hospital, physician office and home health care personnel work together to transition patient care and reduce readmissions. Delivery of care also includes a cloud-based EMR system using Android phones to make patient information readily available. The system provides care pathways based on each patient’s condition and a physician portal offers real-time clinical information in a HIPAA-secure environment.
